Gear Issue and Return Agreement

This agreement must be signed by all upon receiving any equipment issued during a training program or assignment.

    *Responsibility for Equipment:
    I, the undersigned, acknowledge that I am fully responsible for the proper care, handling, and use of the equipment issued to me during the training program or assignment. I agree to take all reasonable steps to protect and maintain the equipment in good working condition.

    *Proper Use:
    I understand that the equipment issued is to be used only for the intended purposes within the training program or assignment. I will not modify, alter, or misuse the equipment in any way.

    *Return of Equipment:
    I agree to return the equipment at the end of the training program or assignment, or earlier if requested by the training staff or project manager. The equipment must be returned in the same condition as it was issued, subject to normal wear and tear.

    *Damage or Loss:
    I understand that I will be held financially responsible for any damage to or loss of the equipment issued to me, beyond reasonable wear and tear. If the equipment is damaged, lost, or stolen, I will notify the training staff or project manager immediately. I acknowledge that any costs for repair or replacement of the equipment will be my responsibility.

    *Inspection of Equipment:
    Upon returning the equipment, I understand that it will be inspected by training staff or project manager to assess its condition. If any damage or loss is found, I will be informed of any required payment for repairs or replacement.

    *Failure to Return Equipment:
    I acknowledge that failure to return the issued equipment by the end of the training program, or assignment, or upon request may result in penalties, including withholding of my training certification and/or legal action to recover the value of the equipment.
